Key Responsibilities- Provide outreach and care coordination to facilitate smooth transitions between mental health providers and different levels of care.
- Assess and triage referred cases, determining the level of care needed and providing effective communication of findings and recommendations to care providers.
- Develop and document patient-centered plans of care in collaboration with members, families, and care teams.
- Provide brief interventions using evidence-based techniques to promote patient engagement, self-management, and recovery.
- Maintain thorough knowledge of Kaiser Permanente services, benefits, and community resources.
- Minimum one year of previous social work experience.
- Masters degree in social work (MSW).
- Licensed Independent Clinical Social Worker (Washington) required at hire.
- Minimum three years of experience post licensure in hospital, outpatient clinic, or other mental health agency.
- Working knowledge of KPWA services and benefits.
- Expertise with brief interventions and case management techniques.
