Position Overview:


The LPN Resource Coordinator will prioritize the health and safety of all clients while delivering essential nursing care.

Reporting to the Regional Director of Operations, this role will provide nursing support to various communities within the local market, addressing staffing shortages as they arise.

The LPN Resource Coordinator will collaborate with the Wellness Director to plan, coordinate, and oversee home care services, ensuring compliance with state regulations and company policies.

This position is responsible for the overall management, coordination, and monitoring of care service delivery under the guidance of the Wellness Director.


Key Responsibilities:
  • Ensure compliance with federal, state, and local regulations and standards.
  • Maintain the confidentiality and privacy of clients and their records.
  • Be available for staffing notifications and carry a cell phone.
  • Administer prescribed medications to residents.
  • Monitor residency requirements and recommend alternative placements when necessary.
  • Assist care staff as needed and maintain accurate client charts and nursing notes.
  • Communicate any changes in client conditions to physicians promptly.
  • Oversee medication orders and renewals, ensuring follow-up with physicians as required.
  • Maintain client files and necessary documentation.
  • Ensure clients can safely reside in their units, recommending additional care and services when needed.
  • Foster positive working relationships with Capital Senior Living staff, providing support as necessary.
  • Maintain open communication with clients, families, and team members.
  • Address client concerns or complaints effectively.
  • Keep the Wellness Director informed of any needs or issues.

Qualifications:
  • Must hold a valid LPN license in the state of Indiana, maintaining current and unencumbered licensure.
  • A valid driver's license is required if driving is part of the role.
  • Ability to understand and apply relevant federal, state, and local regulations to ensure compliance.
  • Demonstrated ability to support and address the needs of programs and services.
  • Effective participation and collaboration as a team member are essential.
  • This position requires travel, with a significant portion of time spent on the road.
