Job Summary

We are seeking a skilled Licensed Practical Nurse (LPN) to join our Home Health team at Indiana Fitness Works Inc. As a Home Health LPN, you will provide high-quality patient care and perform technical skilled care in the patient's home.

Key Responsibilities:

  • Provide nursing care according to the plan of care and in coordination with other members of the healthcare team.
  • Visit patients and provide nursing care as assigned.
  • Participate in the in-service program and complete progress notes for each visit.
  • Continually observe and report patients' nursing needs and instruct patients' family/caregivers in care and maintenance of a healthy environment.
  • Adhere to and participate in the Care Management process.

Requirements:

  • Current LPN licensure in the state of Indiana.
  • Current CPR certification.
  • Current driver's license, vehicle insurance, and access to a dependable vehicle.

Preferred Qualifications:

  • Home health experience.
  • Full vaccination against Covid-19 or a medical or religious exemption.
