We are seeking a Clinical Care Coordinator to become an integral part of our call center team. If you are a proficient RN or LPN with a commitment to patient-centered care and possess triage experience in a clinical environment, we would like to hear from you.

Role Summary

As a Clinical Care Coordinator, you will play a crucial role in enhancing the patient experience from the first point of contact through to their visit. You will thrive in a collaborative atmosphere that prioritizes teamwork and aligns with IHC's mission.

Key Responsibilities
  • Improve patient interactions from initial inquiries to appointment completion, including managing referrals to specialists.
  • Support the onboarding and training of new personnel within the Patient Service Center.
  • Handle incoming calls, schedule appointments, direct inquiries, and address patient concerns.
  • Oversee referrals, complete prior authorizations, arrange diagnostic imaging, and manage related documentation.
  • Work closely with management to schedule appointments in accordance with IHC's guidelines and requirements.
  • Ensure timely processing of patient documentation.
  • Inform patients about necessary paperwork for their appointments.
  • Maintain and retrieve patient data accurately using IHC's Electronic Medical Records (EMR) system.
  • Convey diagnostic and clinical information effectively to patients.
  • Coordinate with the patient care team as needed.
  • Assist with reminder calls, follow-ups, and pre-chart preparations to enhance operational efficiency.
Preferred Qualifications
  • Experience in triage within a clinical setting.
  • Familiarity with pre-authorization processes.
  • Adherence to established work practices and procedures.
  • Strong attention to detail and accuracy.
  • Ability to perform tasks efficiently and within deadlines.
  • Knowledge of standard office practices and procedures.
  • Capability to manage multiple requests simultaneously.
  • Engagement in process improvement initiatives and teamwork.
  • Proficient in office technology and software, including data entry, Microsoft Office Suite, eCW, and Dialpad.
