Position Overview

The LTSS SERVICE COORDINATOR – RN CLINICIAN plays a crucial role in managing the care of members within their professional licensure. This position involves guiding non-RN clinicians in accordance with state regulations and contractual obligations. The primary objective is to develop, monitor, assess, and refine the care plans tailored to meet the unique needs of each member, thereby enhancing healthcare delivery across various stages.

Key Responsibilities:

  • Conduct comprehensive clinical evaluations through telephonic or in-person interactions to identify and address the physical, mental, and social service needs of members.
  • Identify members at risk for complications and facilitate coordinated care with the healthcare team.
  • Oversee the management of members with chronic conditions, co-morbidities, and disabilities to ensure optimal utilization of healthcare benefits.
  • Compile detailed member histories to inform personalized care strategies.
  • Collaborate with members, their families, caregivers, and healthcare providers to establish care goals and identify appropriate service levels or waiver programs.
  • Formulate care plans that guarantee access to necessary services for members.
  • Assist in the execution of care plans by managing authorizations and referrals within the benefits framework.
  • Work in partnership with Medical Directors and Inter-Disciplinary Teams to develop effective care management strategies.
  • Address and resolve any issues related to providers, claims, or services.
  • Supervise LPN/LVN, LSW, LCSW, LMSW, and other licensed professionals in the coordination of member services.

Qualifications:

  • Registered Nurse (RN) with a minimum of 3 years of experience in service coordination or case management for individuals with chronic illnesses, co-morbidities, or disabilities; or equivalent education and experience.
  • Possession of a current, unrestricted RN license in the relevant state(s).

Preferred Qualifications:

  • Master's degree in Health or Nursing is preferred.
  • State-specific certification may be required in accordance with legal and contractual standards.
  • Willingness to travel to various work sites as necessary.
