Key Responsibilities:
- Deliver direct patient care in accordance with the State Nurse Practice Act
- Implement current nursing practices based on thorough assessments and care plans
- Maintain accurate and timely documentation of patient services reflecting the care plan
- Educate patients and families/caregivers regarding diagnoses and self-care plans
- Coordinate hospice services, effectively communicating identified needs to the interdisciplinary team
- Manage pain and symptoms, evaluating patient responses to treatments and medications

Qualifications:
- Graduate of an accredited practical nurse or vocational nursing program
- Minimum of three years nursing experience, preferably in community health, hospice, or medical/surgical settings
- Current licensure as an LPN/LVN in the practicing state
- Valid CPR certification
- Strong verbal and written communication and organizational skills
- Valid driver's license with insured automobile as per state requirements
- Understanding of hospice philosophy and the needs of terminally ill patients
