Housing Stability Case Manager

Community of Hope is seeking a dedicated and compassionate Housing Stability Case Manager to join our team. As a key member of our Rapid Rehousing program, you will play a vital role in supporting families and individuals in gaining and maintaining housing stability.

Key Responsibilities:
  • Develop and implement case management plans that address the unique needs of each family or individual, focusing on core areas such as housing, income, and family stabilization.
  • Meet with clients in their homes to review progress, address barriers, and provide support and guidance.
  • Collaborate with families and Housing Specialists to facilitate the housing search and placement process, including accompanying clients to view units and advocating on their behalf.
  • Network with community resources to connect clients with essential services, such as employment, emotional wellness, and training opportunities.
  • Engage with clients to assess their needs, preferences, and strengths, and provide empathetic and flexible support to foster independence.
  • Act as a liaison with landlords and other community supports as needed, ensuring seamless communication and coordination.
  • Participate in multidisciplinary meetings, trainings, and regular supervision to stay informed and aligned with program goals.
Requirements:
  • Bachelor's Degree with a minimum of 1 year of experience in case management, or equivalent combination of education and experience.
  • Strong knowledge of community resources and ability to become knowledgeable required.
  • Strong interpersonal and organizational skills, with excellent verbal and written communication skills.
  • Proven ability to submit timely, detailed, and accurate case notes.
  • A valid driver's license and regular access to a car, as well as proof of auto insurance, required for staff working in scattered site housing programs.
