About the Role

We are seeking a passionate and dedicated individual to join our team as a Housing Stability Case Manager. In this role, you will play a critical part in supporting families in gaining and maintaining housing stability through developing case management plans that incorporate goals for both adults and children related to core areas: finding stable housing, increasing income, and family stabilization and support.

Key Responsibilities
  • Develop and implement case management plans that address the unique needs of each family, including housing search and placement, income increase, and family stabilization and support.
  • Meet face-to-face with families on caseload to pursue goal-related tasks, review progress, and address barriers to accessing services.
  • Network with community resources for housing, employment, emotional wellness services, training, etc., and connect families as appropriate.
  • Maintain regular communication with all families on caseload and document progressive engagement efforts with difficult-to-engage families.
  • Work consistently within a housing-first framework, ensuring that services offered are strengths-based and client-centered.
Requirements
  • Associate's degree or two years of college.
  • In lieu of the education requirement, a minimum two years of experience with homeless services programs or provision of social services is required.
  • Bachelor's degree preferred.
  • BSW and MSW degree holders must have a LSWA/LGSW or appropriate social work license prior to starting.
  • Minimum of two years of experience working in housing/homelessness or social services field.
  • Strong knowledge of community resources or ability to become knowledgeable required.
  • Strong interpersonal and organization skills required.
  • Strong communication skills including verbal and writing.
  • Proven ability to submit timely, detailed, and accurate case notes.
  • Bilingual: Fluent in English and Spanish Preferred.

Community of Hope is a mission-driven, innovative, and rapidly growing nonprofit. For over 40 years, we have provided healthcare, housing, and supportive services for under-resourced, underserved, and people experiencing homelessness in Washington, DC. As a Federally Qualified Health Center, we provide medical, dental, emotional wellness, and care coordination services for the whole family at three locations in DC. Community of Hope also strongly emphasizes maternal and child health, with midwifery practice and the only free-standing birth center in DC.
