Job Summary:

Soliant Health is partnering with a leading healthcare organization to fill a full-time Clinical Pharmacist position. As a key member of the pharmacy team, you will be responsible for providing clinical expertise and patient care services.

Key Responsibilities:

  • Documenting Drug Information: Accurately document drug information questions, drug utilization evaluation activities, and adverse drug reactions.
  • Patient Care: Understand and respond effectively to the unique needs of neonatal, pediatric, adolescent, adult, and/or geriatric patients, as demonstrated by effective documentation in progress notes and pharmacy care plans.
  • Leadership and Guidance: Ensure adherence by pharmacy interns, pharmacy residents, and pharmacy technicians to departmental policies and procedures, as well as State and Federal laws and regulations.
  • Formulary Management: Promote adherence to the approved Drug Formulary by the Pharmacy and Therapeutics Committee of the Medical Staff.
  • Charging and Documentation: Ensure proper charging documentation is completed correctly for services provided.
  • Community Outreach: Promote and market the pharmacy department to the community, patients, hospital employees, members of the medical staff, and their office employees in an informal manner.
  • Communication: Maintain a positive, direct, open communication line with members of the Medical Staff and their employees.
  • Clinical Competency: Maintain clinical competency through successful completion of self-study modules and providing clinical services in practice.
  • Teaching and Supervision: Provide teaching, direction, and supervision of pharmacy students, residents, and medical assistants working under your direction.
  • Professional Development: Participate in and support professional organizations.
  • Program Development: Take an active, participating role in the development and implementation of new programs, while supporting and facilitating programs in place.
  • Competency Assessments: Complete assigned competency assessments.
  • Additional Responsibilities: Complete additional responsibilities/projects when assigned by Director.

Preferred Qualifications:

Completion of an advanced degree is preferred, but not required. Completion of an ASHP Accredited Residency and/or Fellowship is also preferred, but not required.

Required Experience:

Demonstrated experience in the following areas: Anticoagulation therapy, bridge therapy, and drug information.
