Job Summary

Soliant Health is seeking a highly skilled Clinical Pharmacist to join our team. As a Clinical Pharmacist, you will play a critical role in providing exceptional patient care and contributing to the success of our healthcare organization.

Key Responsibilities
  • Documenting and Reporting: Accurately document drug information questions, drug utilization evaluation activities, and adverse drug reactions to ensure seamless communication among healthcare professionals.
  • Patient-Centered Care: Demonstrate a deep understanding of the unique needs of patients across various age groups, including neonatal, pediatric, adolescent, adult, and geriatric populations, and provide effective documentation in progress notes and pharmacy care plans.
  • Leadership and Guidance: Ensure adherence to departmental policies and procedures, as well as State and Federal laws and regulations, by pharmacy interns, residents, and technicians under your supervision.
  • Formulary Management: Promote adherence to the approved Drug Formulary by the Pharmacy and Therapeutics Committee of the Medical Staff.
  • Charging and Billing: Ensure accurate and complete charging documentation for services provided.
  • Community Outreach: Promote and market the pharmacy department to the community, patients, hospital employees, and medical staff in an informal manner.
  • Communication and Collaboration: Maintain a positive, direct, and open communication line with members of the Medical Staff and their employees.
  • Clinical Competency: Maintain clinical competency through successful completion of self-study modules and provision of clinical services in practice.
  • Teaching and Supervision: Provide teaching, direction, and supervision of pharmacy students, residents, and medical assistants working under your direction.
  • Professional Development: Participate in and support professional organizations.
  • Program Development: Take an active and participating role in the development and implementation of new programs, while supporting and facilitating existing programs.
  • Competency Assessments: Complete assigned competency assessments.
  • Additional Responsibilities: Complete additional responsibilities and projects assigned by the Director.
Preferred Qualifications
  • Advanced Degree: Completion of an advanced degree is preferred, but not required.
  • Residency and Fellowship: Completion of an ASHP Accredited Residency and/or Fellowship is preferred, but not required.
