Key Responsibilities
  • Provide warm and welcoming greetings to new admissions and families, addressing their questions and concerns.
  • Complete thorough admission nursing documentation and assessments, including capturing patient photos.
  • Keep the nursing team informed of any changes in patient status, ensuring timely communication and collaboration.
  • Work closely with IDTs to develop individualized care plans and participate in family meetings when necessary.
  • Ensure that Attending NPs/MDs are notified of admission and obtain orders promptly.
Requirements
  • Hold an active, unencumbered RN/LPN license in Rhode Island.
  • Bring prior experience in sub-acute and/or long-term care settings.
  • Exhibit excellent communication and teamwork skills.
  • Be fully vaccinated against COVID-19.
