About This Opportunity

We are seeking a passionate and dedicated Registered Nurse to provide skilled assessments and complete admissions for eligible patients in their homes and long-term care facilities.

What You'll Do
  1. Provide skilled assessments to determine patient eligibility for hospice services.
  2. Complete admissions for eligible patients in compliance with Medicare/Medicaid and state regulations.
  3. Develop individualized plans of care including documenting Local Coverage Determination.
