Job Summary

We are seeking a highly skilled Licensed Practical Nurse (LPN) to join our healthcare team at Lifespan. As an LPN, you will be responsible for providing direct and indirect patient care, health teaching, and delegating duties to other caregivers as required.

Duties and Responsibilities
  • Conduct assessments of patients' health status by collecting and recording data from various sources, including documentation, observation, and patient interviews.
  • Participate in nursing diagnosis and patient needs based on assessment data, including physical, psychosocial, cultural, educational, and spiritual needs.
  • Identify expected patient outcomes/goals and initiate discharge planning on admission in collaboration with the patient/significant other and the multi-disciplinary treatment team.
  • Document planned patient discharge instructions and services on the discharge instruction sheet and complete patient referral forms for post-discharge services.
  • Provide input into patient or caregiver learning needs and document an educational plan that considers the patient's age, cognitive level, culture, and preferred method of learning.
  • Administer therapeutic interventions safely and efficiently, including medication administration, observing patient response, and documenting in the medical record.
  • Perform limited I.V. therapy procedures and assist Licensed Independent Practitioners with treatments, procedures, and special tests as required.
  • Delegate selected patient care activities to other licensed practical nursing staff and/or unlicensed personnel, ensuring appropriate supervision and training.
Requirements
  • Graduate of School of Practical Nursing.
  • Current licensure as a Licensed Practical Nurse in the Commonwealth of Massachusetts; Basic Life Support Certification.
Salary and Benefits
