Position Overview

The Clinical Nurse Educator is responsible for overseeing the integration of services between the health care team and the community, ensuring that both existing health conditions and preventive health measures are addressed through a comprehensive health strategy.

This role guarantees that clients have access to essential services related to mental health and substance use, while also tracking and reporting on any barriers to access.

Key Responsibilities

- Facilitate connections between clients and available resources, providing referrals to additional programs as necessary.

- Supervise Peer Support Specialists and conduct organization-wide training on integrated care services.

- Coordinate services between the health care team and community resources to ensure comprehensive care.

- Organize and maintain regular collaborative meetings with system partners to enhance patient care and meet benchmark standards.

- Educate clients on health-related issues pertinent to their care, including community resources, insurance options, and financial assistance.

- Engage in Quality Improvement initiatives, including data collection and analysis, to enhance service quality.

- Oversee the work of staff and interns, providing constructive feedback and performance evaluations.

- Deliver professional, recovery-oriented nursing care by conducting assessments of clients' physical and behavioral health, formulating care plans, and evaluating responses to interventions.

- Maintain oversight of medication samples and ensure accurate record-keeping of medications and controlled substances.

- Collaborate with the Health Home Team and providers to identify health needs and develop care coordination plans.

- Create informational resources for clients and staff regarding mental and physical health services.

- Develop and coordinate wellness programs, including tracking participation and evaluating outcomes.

- Participate in Quality Assurance and Quality Improvement processes to ensure compliance with state regulations and organizational policies.

Qualifications

- Current Registered Nurse Certification in the State of Texas.

- Minimum of two years of nursing experience.

- Additional certifications such as LPC or LMSW are preferred.

- Must possess a valid driver's license and maintain required credentials.

Preferred Skills

- Bachelor's degree in Nursing from an accredited institution.

- Experience in case management and knowledge of trauma-informed care.

- Bilingual abilities (English/Spanish) are advantageous.

Physical Requirements

- Ability to meet physical demands associated with training and client care, including lifting and mobility tasks.

- Must be capable of frequent walking, standing, and bending as part of job responsibilities.
