Position Overview

The Clinical Nurse Educator plays a vital role in bridging services between the Integrated Care team and the community, ensuring that both existing health conditions and preventive health initiatives are addressed within a comprehensive health framework.

This role guarantees that clients have access to services related to mental health and substance use, while also monitoring and reporting on barriers to access.

Responsibilities include connecting clients with essential resources and providing referrals to additional programs. The Clinical Nurse Educator supervises Peer Support Specialists and conducts organization-wide training on Integrated Care services and group initiatives.

Key Responsibilities

- Facilitate coordination of services between the Integrated Care team and community resources to address both current health issues and preventive care needs.

- Organize and maintain regular collaborative meetings with Integrated Care and system partners to enhance patient care and meet benchmark standards.

- Educate clients on health matters pertinent to their care, including community health resources, insurance options, financial assistance, and pharmaceutical services.

- Engage in Quality Improvement initiatives, which encompass data collection, tracking, and analysis.

- Oversee the work of staff and interns, providing constructive feedback and performance evaluations.

- Deliver professional, recovery-oriented nursing care by conducting assessments of clients' physical and behavioral health, formulating nursing diagnoses, developing care plans, and evaluating responses to interventions.

- Maintain oversight of medication samples for designated units, ensuring accurate inventory of stock medications and controlled substances.

- Collaborate with the Health Home Team and treating providers to identify health and wellness needs and develop care coordination plans to address treatment barriers.

- Create informational resources for clients and staff on mental health, physical health, and integrated health services.

- Coordinate wellness programming, including promoting activities, tracking attendance, and evaluating outcomes.

- Participate in Quality Assurance and Quality Improvement activities, ensuring culturally responsive and trauma-informed services.

Minimum Qualifications

- Current Registered Nurse Certification in the State of Texas.

- A minimum of two years of nursing experience.

- Must possess a valid driver's license and automobile insurance, with the ability to travel as required.

Preferred Qualifications

- Bachelor's degree or higher in Nursing from an accredited institution.

- Previous experience in case management is advantageous.

- Bilingual proficiency (English/Spanish) is preferred.

- Familiarity with wellness-oriented, trauma-informed care practices.

- Prior experience in a supervisory role is preferred.

Competencies for Success

- Knowledge of federal, state, and local regulations in nursing practice.

- Skills in mediation, counseling, organization, and time management.

- Ability to communicate effectively, both verbally and in writing, and to maintain accurate records.

- Physical demands include the ability to meet training requirements and perform essential nursing duties.
