Job Summary

We are seeking a highly skilled Clinical Pharmacist to join our team at Trinity Health. As a key member of our pharmacy department, you will play a critical role in providing exceptional patient care and medication management services.

Key Responsibilities
  • Review medication orders to ensure appropriateness of therapy, including dosage form, dose, and route of administration, schedule, starting time, and duration.
  • Monitor patient medication regimens for possible allergies, drug interactions, nutrient-drug interactions, contraindications, and potential adverse drug reactions.
  • Individualize medication therapy based on patient variables, including renal function, age, weight, laboratory values, and pharmacokinetic parameters.
  • Manage drug therapy and utilization across the continuum of care, documenting clinical interventions, participating in quality assurance monitors, drug utilization review, pharmacoeconomic analysis, and monitoring medication therapy outcomes.
  • Perform selective patient medication histories and counseling, providing education materials as required.
  • Consult with and advise physicians and other healthcare professionals in the development of safe, rational, and cost-effective patient medication care plans.
  • Respond promptly and thoroughly to requests from healthcare personnel for patient-specific pharmacotherapeutic information.
  • Direct and monitor pharmacy technicians, interns, aides, volunteers, and students in preparation, packaging, labeling, and distribution of medication.
  • Direct, monitor, and verify the compounding of injectable medications, including epidural infusions, irrigation, small and large volume parenterals, hyperalimentation, and cytotoxic agents performed by pharmacy technicians.
  • Compounds and dispenses medications and injectables, recognizing that compounding and/or dispensing errors may result in significant patient morbidity and mortality. Competency must be demonstrated before compounding or checking compounded products. Adheres to USP 797/USP 800 Guidelines.
  • Participate in hospital emergency codes responses. Provides and prepares emergency medications via emergency carts, cardiac kits, and EMS/Ambulance kits.
  • Coordinate pharmaceutical services for specified hospital units. Problem-solve, communicate with, and serve as a liaison between these units and the pharmacy department.
  • Initiate, direct, coordinate, and perform specific departmental projects.
  • Adhere to and enforce Pharmacy & Therapeutics and departmental policies and procedures.
  • Participate in teaching programs affiliated with the Department of Pharmacy and training programs for nursing, physicians, pharmacists, and other allied health professionals.
  • Perform duties in compliance with all federal, state, and local regulatory requirements.
Requirements
  • Requires a Bachelor's degree in Pharmacy. A Pharm.D. or other related advanced degree is desirable. A current State of Michigan Pharmacist license or eligibility for licensure. Maintenance of 30 continuing education credits every two years as required by the State Board of Pharmacy.
  • Previous related experience or hospital residency is desirable.
Skills and Abilities
  • Broad knowledge of medications compounding and dispensing.
  • Thorough knowledge of pathophysiology, pharmacokinetics, and therapeutics.
  • Interpersonal skills necessary to interact with patients, physicians, and other healthcare providers.
  • Analytic skills necessary to organize and prioritize multiple job duties.
  • General understanding of computers, including general operations and data retrieval.
  • Communication skills necessary for clear written and oral communication.
  • Thorough knowledge of the regulatory, statutory, and professional aspects of pharmacy practice.
