The Clinical Pharmacist Specialist in Pain Management and Palliative Care plays a vital role in enhancing patient outcomes through specialized pharmaceutical care. This position is integral to the development, implementation, and monitoring of pharmacy services tailored to pain management and palliative care needs.
Key Responsibilities
- Patient Care (70% of role)
Provide direct patient care by collaborating with primary and consult teams to optimize medication therapy for patients experiencing pain or requiring palliative care. This includes:- Assessing medication history and conducting medication reconciliation.
- Offering expert recommendations on medication selection and dose adjustments.
- Engaging with multidisciplinary teams to ensure a cohesive approach to patient care.
- Education (20% of role)
Facilitate educational initiatives related to pain management, including:- Developing training programs for healthcare professionals.
- Serving as a resource for pain management education for patients and caregivers.
- Research (10% of role)
Participate in research activities and quality improvement projects that align with the goals of the healthcare system, focusing on:- Evaluating drug use and cost management strategies.
- Contributing to the advancement of pharmacy services in pain management.
The ideal candidate will possess:
- A Doctor of Pharmacy degree or equivalent.
- Completion of a pharmacy residency program or equivalent experience.
- Current licensure as a registered pharmacist.
- Strong communication skills and the ability to work collaboratively within a healthcare team.
This position may require flexible scheduling, including evenings and weekends, to meet departmental needs. Opportunities for hybrid or remote work may be available at the discretion of the department.
