Position Summary

The Resident Care Coordinator plays a vital role in delivering empathetic assistance and expert advice to enhance the well-being and quality of life for residents. This position involves conducting assessments, strategic planning, and facilitating access to resources and services. The Coordinator collaborates with various teams to ensure residents meet essential living conditions and thrive within the community. Key Responsibilities

  • Facilitates connections for new residents to the Horizon House community, establishing rapport and outlining available services from both internal and external partners, including healthcare and support groups.
  • Ensures residents are informed about safety protocols, including emergency response systems.
  • Collaborates with residents to assess needs, plan, implement, and document necessary support and resources from community and external partners, such as home care and mental health services.
  • Assists residents in maintaining accurate and current documentation, including medical and legal information.
  • Provides backup support for Assisted Living residents facing mental health or emotional challenges.
  • Works with interdisciplinary teams to understand resident needs and provide appropriate support.
  • Evaluates resource utilization to balance quality care with financial considerations.
  • Advises residents and families on transition issues, end-of-life choices, and legal matters, serving as a resource for complex inquiries.
  • Respects and supports residents' end-of-life decisions, including options for dignified death.
  • Manages care continuity through key performance indicators and prepares monthly reports.
  • Coordinates financial assessments and admission processes for prospective Assisted Living residents.
  • Supports transitions for residents moving from Independent Living to Assisted Living, ensuring a seamless experience.
  • Collaborates with the Assisted Living Team to facilitate the return of short-term residents to Independent Living.
  • Provides input for resident assessments and collaborates with various departments on resident needs.
  • Engages with the Community Engagement Manager to identify relevant referrals and outreach opportunities.
  • Builds relationships with external educational and resource organizations related to aging and mental health.
  • Delivers presentations on significant topics for residents and staff.
  • Maintains a balance between advocating for resident concerns and upholding the organization's mission.
  • Establishes connections with hospital discharge planners to ensure alignment with living conditions for returning residents.
  • Embodies the vision, mission, and values of Horizon House.
  • Represents Horizon House with professionalism and positivity.

Additional Responsibilities

  • Participates in community initiatives and meetings as requested.
  • Engages in ongoing education and networking opportunities relevant to the role.
  • Identifies and develops programs or services to address the evolving needs of residents.
  • Completes additional tasks and projects as assigned by management.

Qualifications

  • Bachelor's or Master's Degree in Social Work, Occupational Therapy, or Nursing from an accredited institution, or a minimum of five years' experience as a Certified Geriatric Care Manager.
  • Experience working with older adults, including knowledge of dementia and complex family dynamics.
  • Familiarity with Assisted Living and interdisciplinary approaches, along with knowledge of community services.
  • At least three years of experience serving older adults in a community-based environment.
  • Proficient in Windows-based computer systems and Microsoft Office applications.
