POSITION SUMMARY: Aperion Care Lakeshore is seeking a dedicated Registered Nurse responsible for delivering direct nursing care to residents while overseeing the daily nursing operations conducted by nursing assistants. This role requires adherence to current federal, state, and local standards, guidelines, and regulations governing our facility, as well as directives from the Director of Nursing to ensure the highest quality of care is consistently maintained.

KEY RESPONSIBILITIES:

  • Oversee the daily activities of nursing assistants. Collaborate with assigned nursing staff and support personnel to plan services, programs, and activities for the shift. Prepare written and oral reports regarding shift activities as necessary. Manage the admission, transfer, and discharge processes for residents. Provide leadership to nursing staff assigned to your unit/shift.
  • Complete and maintain required documentation upon resident admission, transfer, and discharge. Accurately receive and transcribe physician orders, recording them on the appropriate forms. Document nursing notes in a detailed manner that reflects the care provided and the resident's response. Complete incident reports and submit them as required. Perform routine charting duties in accordance with established policies and procedures.
  • Administer medications as prescribed by physicians.
  • Foster a collaborative working relationship with inter-departmental personnel and other facility departments to ensure nursing services meet residents' needs. Create a warm, engaging atmosphere while maintaining a calm environment throughout the unit and shift. Exchange nursing reports at the beginning and end of shifts. Accompany physicians on rounds as necessary.
  • Provide professional nursing services, including catheterization, tube feedings, suctioning, dressing changes, and other necessary procedures.
  • Monitor nursing staff to ensure compliance with established safety regulations regarding equipment and supplies.
  • Maintain confidentiality of all resident care information, including protected health information. Report any known or suspected breaches of confidentiality. Ensure that nursing care is delivered in privacy, and that nursing staff knock before entering resident rooms.
  • Provide IV care and manage IV sites as needed.
  • Perform additional duties as assigned.

QUALIFICATIONS:

  • Must possess a current, unencumbered state license as a Registered Nurse.
  • Ability to read, write, speak, and understand the English language is essential.
  • Must be capable of making independent decisions when necessary.
  • Knowledge of nursing and medical practices, as well as relevant laws, regulations, and guidelines for nursing care facilities, is required.
  • Current CPR certification is mandatory.
  • Experience in Long Term Care is preferred.

PHYSICAL REQUIREMENTS:

  • Ability to move throughout the workday.
  • Must manage the mental and emotional demands of the position.
  • Must meet the general health requirements set forth by facility policies, including medical and physical examinations.
  • Ability to push, pull, move, and/or lift a minimum of 25 pounds to a height of at least 5 feet and carry such weight a minimum distance of 50 feet.
  • May be required to assist in evacuating residents during emergencies.
