Role Overview: As a pivotal member of the population health team, the Clinical Pharmacy Team Leader will oversee the transitional care pharmacy unit, directly managing a group of clinical pharmacists. This role will be instrumental in leading the clinical pharmacist initiatives within the organization, particularly in the Medication Reconciliation Post Discharge (MRP) program. The Team Leader will also mentor and guide pharmacists, contributing to strategic pharmacy initiatives and professional development.

Key Responsibilities:
  • Lead and manage a team of Clinical Pharmacists.
  • Design, implement, and oversee clinical pharmacy programs focusing on transitional care medication evaluations.
  • Establish collaborative practice agreements to enhance clinical pharmacy functions across multiple states.
  • Utilize data analytics to monitor, benchmark, and report on the effectiveness of clinical pharmacy interventions.
  • Assist the pharmacy team in reviewing Discharge Summaries and engage with patients and healthcare providers to ensure comprehensive medication reconciliation.
  • Assess post-discharge medication orders and profiles to mitigate risks such as drug interactions and allergies.
  • Ensure appropriate drug therapy based on patient diagnoses and promote cost-effective medication choices.
  • Collaborate with primary care physicians to develop patient care plans.
  • Provide recommendations to medical teams based on clinical evidence and personal expertise.
  • Communicate recommendations clearly and effectively, considering patient-specific factors.
  • Conduct patient interviews to gather medical histories and evaluate responses to therapies.
  • Analyze laboratory data to formulate treatment plans and monitor therapeutic responses.
  • Apply pharmacokinetic principles to develop safe and effective medication regimens.
  • Address therapeutic inquiries from healthcare providers.
  • Identify and resolve drug-related issues, ensuring patient safety.
  • Implement procedures to detect and prevent significant drug interactions.
  • Facilitate continuity of care by collaborating with other healthcare professionals.
  • Educate patients and caregivers on medication administration and potential side effects.
  • Engage in performance improvement initiatives, including data collection and evaluation.
  • Communicate organizational updates to pharmacy team members and escalate concerns as necessary.
  • Assist in onboarding new pharmacists and foster a culture of professional growth.
  • Conduct performance evaluations and support team members in achieving their professional goals.
  • Perform additional duties as required.

Qualifications: We seek a dedicated and experienced clinical pharmacist with:
  • Current pharmacist licensure in good standing in at least one of the following states: IL, IN, MI, PA.
  • A minimum of three years of experience in medication therapy management or related fields.
  • Demonstrated leadership experience in guiding and mentoring healthcare providers.
  • Exceptional customer service and communication skills.
  • Proficiency in MS Office and Google Suite.
  • Ability to analyze complex data for informed decision-making.
  • Board certification and advanced degrees are advantageous but not mandatory.
  • Authorization to work in the United States.
