CenterWell Home Health is in search of a Manager of Care Management Support to oversee the administration of care management services. This role is pivotal in ensuring the well-being of our members through effective resource management and support.

  • Facilitates the assessment and evaluation of members' health needs, guiding them and their families to appropriate resources for optimal wellness.
  • Responsible for strategic decision-making regarding resource allocation and operational tactics, requiring collaboration across various departments.
  • Conducts regular briefings and meetings, maintaining consistent communication with other management personnel.
  • Reviews and adjusts processes to ensure compliance with contractual obligations.
  • Provides continuous coaching and constructive feedback to enhance team performance and competency.
  • Stays informed about Medicaid benefits and services relevant to our members.
  • Operates independently while also collaborating with a team to achieve objectives.
  • Applies advanced technical knowledge to address moderately complex challenges, determining the best approach and resources needed.

Required Qualifications

  • Must reside within Virginia or within a 40-mile radius of the state.
  • Bachelor's degree required.
  • A minimum of three years of professional experience in care management, particularly in addressing healthcare needs for Medicaid and/or Medicare members.
  • At least two years of experience in a management or supervisory role.
  • Proficient in Microsoft Office Suite, including Word, Excel, and PowerPoint.
  • Strong analytical skills with the ability to interpret data trends.
  • Experience in business consulting or operational leadership.
  • Excellent organizational and communication skills, with the ability to conduct meetings effectively.
  • Familiarity with electronic medical records and various information applications.
  • Exceptional interpersonal skills, capable of building rapport with both internal and external stakeholders.
  • This position involves direct interaction with patients and requires participation in health screening programs.
  • Willingness to travel as necessary.
  • A valid driver's license and proof of vehicle insurance are required.

Preferred Qualifications

  • Master's degree preferred.
  • Experience with Long-Term Services and Supports (LTSS) populations or in managed care.
  • Five or more years of supervisory experience.
  • Knowledge of community health resources and social service agencies.
  • Bilingual candidates are encouraged to apply.

Additional Information

  • Workstyle: This position is remote but requires travel.
  • Travel: Up to 25% for collaboration and field interactions.
  • Workdays and Hours: Monday to Friday; standard business hours.
  • Language Proficiency Testing: Candidates may be required to undergo language proficiency assessments.
