Key Responsibilities- Lead Care Management Teams: Supervise and manage a team of care coordinators and other healthcare professionals to ensure that patients receive comprehensive and coordinated care.
- Develop and Implement Care Plans: Collaborate with patients, families, and healthcare providers to develop and implement individualized care plans that meet the unique needs of each patient.
- Monitor and Evaluate Care Outcomes: Regularly review and evaluate the effectiveness of care plans and make adjustments as needed to ensure that patients are achieving optimal health outcomes.
- Collaborate with Healthcare Providers: Work closely with healthcare providers, including physicians, nurses, and other healthcare professionals, to ensure that patients receive seamless and coordinated care.
- Ensure Compliance with Regulatory Requirements: Ensure that all care management activities are in compliance with relevant laws, regulations, and industry standards.
- Master's Degree in Nursing or Related Field: A master's degree in nursing or a related field is required for this role.
- Experience in Care Management: A minimum of 3 years of experience in care management, including experience in leading care management teams and developing and implementing care plans.
- Strong Leadership and Communication Skills: Excellent leadership and communication skills are required for this role, including the ability to motivate and inspire team members and communicate effectively with patients, families, and healthcare providers.
- Knowledge of Healthcare Regulations and Industry Standards: A strong understanding of healthcare regulations and industry standards is required for this role, including knowledge of relevant laws, regulations, and standards related to care management.
