Care Management Support Manager

Position Overview:
We are seeking a dedicated Manager of Care Management Support to oversee the administration of care management services. This role is essential in ensuring that our members receive the appropriate resources and support for their health and wellness needs.
Key Responsibilities:
- Facilitate the assessment and evaluation of members' health requirements to promote optimal wellness.
- Guide members and their families in accessing necessary resources for their care and well-being.
- Make informed decisions regarding resource allocation and operational strategies within the department.
- Conduct regular briefings and maintain communication with other management personnel to ensure alignment and collaboration.
Qualifications:
- Must reside within the specified geographical area.
- Bachelor's degree required; a Master's degree is preferred.
- A minimum of three years of professional experience in care management, particularly within Medicaid or Medicare frameworks.
- At least two years of supervisory or management experience.
Skills:
- Proficient in Microsoft Office Suite and data analysis.
- Strong organizational and communication skills, with the ability to conduct effective meetings.
- Experience with electronic medical records and various information systems.
