Position Overview:

The Care Coordinator plays a pivotal role in orchestrating care planning and service coordination for youth experiencing serious emotional disturbances (SED) under the age of 21.

In this capacity, Care Coordinators ensure that essential services are effectively accessed, organized, and delivered in a manner that is strength-based, individualized, and culturally relevant to the families and youth they serve.

Services and supports are tailored to the unique needs of each youth, developed through a comprehensive Wraparound planning process that culminates in a personalized and adaptable care plan for the youth and their family.

While delivering care coordination, the Care Coordinator collaborates closely with a Family Partner, ensuring that both roles are distinct yet complementary in supporting the youth and their family.

Key Responsibilities:
  • Document detailed progress notes that comply with federal, state, and agency standards.
  • Conduct thorough home-based assessments and behavioral health evaluations, including the age-appropriate Massachusetts Child and Adolescent Needs and Strengths (CANS) assessment, at the family's preferred location.
  • Manage a caseload of 11-14 families, ensuring personalized attention and support.
  • Facilitate the formation of the Care Planning Team and guide the creation of an Individual Care Plan along with a risk management/safety strategy.
  • Monitor the quality and provision of services delivered to families, acting as a liaison for any new services or resources required.
  • Arrange or provide transportation for clients to necessary appointments, crisis interventions, or respite services as needed.
  • Secure support and emergency services for youth and their families when required.
  • Collaborate with other professionals involved in the youth's care to ensure seamless coordination.
  • Identify community resources and foster natural supports for youth and their caregivers to help achieve the goals outlined in the individual care plan.
Candidate Qualifications:

Minimum Experience Required: 1 year

Educational Requirements: A Master's or Bachelor's Degree in an MCE-approved field such as Family Therapy, Counseling and Psychological Services, Human Services, or Community Psychology is required.

A valid driver's license and appropriate auto insurance are mandatory.

Candidates must demonstrate awareness and sensitivity to contextual factors including race, culture, gender, sexuality, disability, economic status, and lived experiences.
