We are seeking an exceptional Care Coordinator to join our team at Pine Grove Crossing Senior Living, a vibrant senior living community in Parker, Colorado. As a Care Coordinator, you will play a vital role in providing exceptional care and services to our residents, ensuring their comfort, safety, and well-being.

Key Responsibilities:
  • Supervise and manage the Assisted Living department, ensuring high-quality care and services are delivered to residents.
  • Develop and implement care plans, working closely with residents, families, and healthcare professionals.
  • Monitor and report any changes or concerns in resident care, ensuring timely interventions and resolutions.
  • Collaborate with the Health and Wellness Director and Executive Director to ensure seamless operations and resident satisfaction.
  • Recruit, train, and develop a high-performing team of caregivers, ensuring they possess the necessary skills and knowledge to provide exceptional care.
  • Manage scheduling, staffing, and resource allocation to ensure efficient and effective care delivery.
  • Ensure compliance with state regulations, company policies, and industry standards.
Requirements:
  • Minimum 4 years of experience in Assisted Living or a related field.
  • Management/Leadership experience, preferably 2+ years.
  • Scheduling experience, preferably 1+ years.
  • CPR and First Aid certification or willingness to obtain within 30 days.
  • Valid, unrestricted certification in the state (CNA desired) and maintenance of CEUs as required by the State.
