Job Summary

We are seeking a dedicated and compassionate RN Case Manager to join our team in delivering exceptional care, comfort, and support to patients.

Responsibilities:
  • Conduct initial assessments of patients and families to determine hospice care needs
  • Regularly re-evaluate patients' nursing needs and develop care plans accordingly
  • Collaborate with the healthcare team, patients, and families to provide continuous and well-coordinated care
  • Counsel patients and families on end-of-life needs and provide support
  • Initiate preventative nursing procedures and communicate with physicians regarding patient needs
  • Supervise Hospice Aides and LPNs
Requirements:
  • Current NJ RN License in good standing
  • Hospice experience preferred
  • Willingness to travel to assigned areas
  • Reliable transportation with liability insurance
  • Strong communication and interpersonal skills
