Position Overview:

As a Clinical Advanced Practice Provider (APP) at Summit Health, Inc, you will play a pivotal role in delivering exceptional healthcare services. This position requires a dedicated Nurse Practitioner or Physician Assistant to work collaboratively within a multidisciplinary team, ensuring comprehensive care for patients.

Key Responsibilities:

Outstanding Patient Care:
  • Delivering personalized and professional healthcare directly to patients.
  • Maintaining precise and up-to-date patient documentation.

Clinical Expertise:
  • Assessing patient symptoms and determining appropriate diagnoses for effective care management.
  • Conducting thorough physical examinations, medication assessments, and mental health screenings.

Collaborative Practice:
  • Partnering with other healthcare professionals, including APPs and physicians, to provide high-quality care in an outpatient setting.
  • Communicating findings to primary care providers to address any identified gaps in patient care.

Qualifications:
  • Graduation from an accredited advanced practice program with certification.
  • A minimum of two years of outpatient clinical experience as an Advanced Practice Provider, demonstrating the ability to work independently.
  • Proficiency in treating adult patients and conducting comprehensive health assessments.
  • Possession of a current, unrestricted state license or eligibility for licensure.
  • Active DEA registration or eligibility for registration.
  • Enrollment in Medicare/Medicaid or eligibility to enroll.
  • Certification in Basic Life Support (BLS) or Advanced Cardiac Life Support (ACLS).
  • Strong communication skills, both verbal and written.
  • Familiarity with Electronic Medical Record (EMR) systems; experience with Athena is preferred.
  • Proficient in Microsoft Office applications.
