As an Advanced Practice Provider with Summit Health, you will collaborate with a multidisciplinary team to deliver exceptional primary care services.
Outstanding Patient Care:
- Delivering personalized and professional healthcare services to patients.
- Ensuring accurate and up-to-date patient documentation.
Clinical Expertise:
- Assessing patient symptoms and determining appropriate diagnoses for effective management.
- Conducting thorough physical examinations, medication assessments, and mental health screenings.
Collaborative Practice:
- Partnering with fellow healthcare providers to ensure optimal patient care in a fully outpatient setting.
- Communicating clinical findings to enhance care coordination.
- Graduation from an accredited advanced practice program and current certification.
- A minimum of two years of outpatient clinical experience as an Advanced Practice Provider, demonstrating the ability to work independently.
- Proficiency in managing adult patients and conducting comprehensive health assessments.
- Possession of a valid state license or eligibility for licensure.
- Active DEA registration or eligibility for registration.
- Enrollment in Medicare/Medicaid or eligibility to enroll.
- Certification in Basic Life Support (BLS) or Advanced Cardiac Life Support (ACLS).
- Excellent communication skills, both verbal and written.
- Familiarity with Electronic Medical Record (EMR) systems; experience with Athena is preferred.
- Proficient in Microsoft Office applications.
