Position Overview


As a Nurse Practitioner or Physician Assistant at Summit Health, you will collaborate with a dedicated care team to deliver exceptional primary care services.

Key Responsibilities

Outstanding Patient Care:

  • Providing personalized, professional care directly to patients
  • Ensuring accurate and up-to-date patient documentation

Clinical Expertise:

  • Assessing patients' current health conditions, establishing diagnoses for effective care management
  • Conducting comprehensive physical exams, medication assessments, and mental health screenings

Collaborative Practice:

  • Partnering with other healthcare providers, APPs, and physicians to deliver high-quality care in a fully outpatient setting
  • Communicating findings to inform primary care providers of any potential care gaps
Qualifications

  • Graduation from an accredited advanced practice program with certification
  • A minimum of two years of outpatient clinical experience as an Advanced Practice Provider, demonstrating the ability to work independently
  • Proficient in treating adult patients and conducting thorough health assessments
  • Possession of a current, unrestricted state license or eligibility for licensure
  • Active DEA registration or eligibility for registration
  • Enrollment in Medicare/Medicaid or eligibility to enroll
  • Certification in Basic Life Support (BLS) or Advanced Cardiac Life Support (ACLS)
  • Strong verbal and written communication skills
  • Familiarity with Electronic Medical Record (EMR) systems; experience with Athena preferred
  • Proficient in Microsoft Office applications
