Outreach Liaison Eldercare Program

At Mount Sinai Medical Center of Florida, we are committed to delivering high-quality healthcare to our diverse community. As a key member of our team, the Outreach Liaison Eldercare Program will play a vital role in promoting our Eldercare program services to eligible patients and families.

Key Responsibilities:
  • Develop and execute strategies to increase enrollment in our Eldercare program, working closely with healthcare system departments to promote services to patients and families.
  • Meet with patients, families, and healthcare professionals to provide education on the PACE model of care and Eldercare program services, and invite those who qualify to enroll in our health plan.
  • Collaborate with the Director of Business Development to develop annual, quarterly, and weekly plans that reflect daily sales calls, staff, and patient engagement.
  • Maintain accurate account lists for all prospective referral sources and track participants through the enrollment process.
  • Attend and participate in Eldercare sales and enrollment meetings, as well as other meetings as directed.
  • Act as a liaison between patients, healthcare systems, and Eldercare to resolve problems, provide information on services, and maintain positive relations.
  • Stay up-to-date on applicable Federal, State, and local laws and regulations, including Mount Sinai and Eldercare policies on compliance, Fraud Waste and Abuse, Code of Conduct, and other policies and procedures.
  • Accept referrals and accurately input information into tracking methodologies and EHRs in a timely manner.
  • Provide timely information on new enrollees to the Interdisciplinary Team and assist as needed to ensure a smooth enrollment process and participant satisfaction.
  • Participate in Interdisciplinary Team meetings to assess potential enrollees for enrollment into the program and provide clarification on information obtained during the enrollment process.
  • Assist with the financial eligibility process for potential PACE participants by reviewing Medicaid eligibility and assisting prospects with filing the Medicaid application, including necessary supporting documentation.
  • Ensure necessary paperwork related to participant enrollment is received by AHCA and DOEA, and Eldercare medical records staff on or before the appointed cut-off date.
  • Collect data and prepare reports in accordance with AHCA, DOEA, DCF, and other agency and CMS requirements.
  • Participate in the quality improvement program.
Qualifications:
  • BLS or CPR certification is required; LPN, BSW, or a Bachelor's degree in a healthcare or marketing-related field is preferred.
  • Two years of sales or marketing experience in a healthcare setting is preferred.
  • At least one year of experience working with the frail or elderly is required.
