Nurse Care Specialist

LHC Group is seeking a skilled Registered Nurse to join our team in providing high-quality care to patients in their homes.
Job SummaryThe Registered Nurse will be responsible for delivering comprehensive nursing care to patients, collaborating with healthcare professionals, and ensuring continuity of care.
Responsibilities- Initiates, develops, and implements individualized care plans in collaboration with physicians and other healthcare professionals.
- Provides direct and indirect supervision to home health aides and licensed practical vocational nurses, ensuring adherence to state and federal regulations.
- Accurately and timely completes comprehensive assessments, including medication reconciliation, and documents patient visits according to policy and payer requirements.
- Communicates effectively with agency staff, physicians, patients, and family members to ensure quality care and service excellence.
- Participates in survey readiness activities, performance improvement plans, and adheres to the agency's utilization management model.
- Current RN licensure in the state of practice
- Current CPR certification
- Current driver's license, vehicle insurance, and access to a dependable vehicle or public transportation
