Job Title: Housing Case Manager

We are seeking a highly skilled and compassionate Housing Case Manager to join our team. As a Housing Case Manager, you will play a critical role in providing comprehensive support and assistance to individuals experiencing homelessness.

Key Responsibilities:

  • Conduct thorough assessments of clients' housing needs, barriers, and goals to develop effective housing plans.
  • Collaborate with clients to identify and access supportive services, such as mental health counseling, substance abuse treatment, and job training.
  • Provide ongoing case management services, including regular check-ins, goal setting, and progress monitoring to ensure clients achieve stable housing and improved well-being.
  • Advocate for clients' rights and access to housing resources within the community, working closely with landlords, property managers, and housing agencies.
  • Maintain accurate and up-to-date client records, progress notes, and case documentation to ensure seamless communication and coordination with team members.
  • Participate in multidisciplinary team meetings and case conferences to coordinate services and support for clients, staying informed about relevant housing policies, programs, and resources.
  • Provide crisis intervention and support during housing transitions or emergencies, demonstrating empathy and patience when working with diverse populations.

Requirements:

  • Bachelor's degree in social work, psychology, counseling, or a related field (Master's degree preferred).
  • Previous experience working with homeless populations or in a housing-focused case management role.
  • Strong understanding of housing issues, homelessness prevention strategies, and housing resources in the local community.
  • Excellent communication skills, both verbal and written, with the ability to collaborate effectively with clients, colleagues, and community partners.
  • Empathy, patience, and cultural competence in working with diverse populations, including individuals with mental health or substance use challenges.
  • Ability to work independently, manage a caseload, and prioritize tasks in a fast-paced environment.
  • Proficiency in Microsoft Office Suite and case management software.
