Job Summary

ASHLIN Management Group, Inc. is seeking a highly skilled and compassionate Case Manager to join our team. As a Case Manager, you will play a critical role in helping clients achieve housing stability and self-sufficiency.

Key Responsibilities
  • Coordinate services and ensure clients are making meaningful progress toward increasing earnings and securing stable housing.
  • Develop and amend Housing Stabilization Plans and DIRPs, help clients overcome barriers to housing stability and self-sufficiency.
  • Build trusting relationships with clients and monitor their progress on the housing stability journey.
  • Conduct outreach to referred clients, engage clients in 2Gen activities, re-engage those clients who started to participate and subsequently stopped.
  • Identify goals and activities appropriate for housing stability, barrier removal, and career-pathways.
  • Coordinate with TEP providers to identify appropriate education and occupational training programs in addition to employment opportunities for families.
Requirements
  • Bachelor's degree in social work, psychology, sociology, counseling, or related service/science or health care related disciplines and one year of experience providing case management services (internship experience applicable).
  • At least two years of work experience in a social service and/or human service field will be considered in lieu of a degree.
  • Bilingual applicants will be recruited to ensure that culturally competent and linguistically appropriate services are provided to individuals and families.
