Program Insight:

The Enhanced Care Management (ECM) initiative is focused on delivering thorough and coordinated support to Medi-Cal beneficiaries who face intricate health and social challenges. This includes individuals with multiple chronic illnesses, those experiencing homelessness, or individuals dealing with significant behavioral health issues. The primary goal is to enhance health outcomes through a personalized approach that encompasses intensive care management, addressing both physical and behavioral health, as well as social determinants of health.

Position Summary:

The ECM LCSW will oversee clinical operations and provide supervision to non-clinical case managers and clinical team members, including associate social workers and/or associate therapists. This role is pivotal in ensuring the provision of high-quality care while adhering to program standards, contractual obligations, and ethical guidelines. The LCSW will act as a clinical resource, guiding the implementation and assessment of care plans aimed at achieving optimal patient outcomes. Collaboration with the interdisciplinary team (ICT) is essential to support patients and engage their support networks in prioritizing their needs and objectives. Furthermore, the LCSW will deliver behavioral health therapy and therapeutic interventions to ECM patients.

Key Responsibilities:

  • Clinical Oversight: Provide supervision to non-clinical case managers and clinical guidance to associate social workers and/or associate therapists.
  • Serve as a clinical resource for the ECM care team.
  • Ensure the delivery of services meets high standards of care.
  • Maintain compliance with program and contractual requirements.
  • Uphold legal and ethical standards while meeting performance expectations.

Care Plan Management:

  • Oversee and evaluate care plans to ensure effective implementation and desired outcomes.
  • Adjust care plans as necessary to meet patient goals.

Patient and Family Support:

  • Offer psychoeducational resources to patients and their families or caregivers.
  • Provide behavioral health therapy and therapeutic interventions.
  • Deliver emotional support and counseling during crises.
  • Facilitate communication and problem-solving to address immediate patient and family concerns.

Training and Development:

  • Conduct training sessions for staff as required.
  • Engage in all assigned meetings and training opportunities.

Collaboration:

  • Work collaboratively with all staff and external partners.
  • Participate in interdisciplinary team meetings to ensure cohesive care.
  • Align closely with ECM administrators, implementing policies and procedures accordingly.

Quality Assurance:

  • Implement quality assurance measures, including record reviews.
  • Support and enforce program policies.
  • Monitor program performance and suggest improvements.
  • Accurately document all services provided in the electronic medical record.
  • Engage in quality improvement initiatives as assigned.

Cultural Sensitivity:

  • Demonstrate awareness of the cultural and socioeconomic characteristics of the service population.

Safety and Compliance:

  • Ensure all work is performed safely and in accordance with safety practices.
  • Adhere to the National Association of Social Workers (NASW) Code of Ethics.
  • Follow all relevant policies, procedures, and legal mandates, including HIPAA and confidentiality regulations.

Education and Qualifications:

  • Master's degree in Social Work (MSW) from an accredited institution.
  • Current and unrestricted Clinical Social Worker (LCSW) license in California.
  • Eligibility to provide clinical supervision to Associate Social Workers (ASW) as per Board of Behavioral Sciences (BBS) guidelines.

Experience and Skills:

  • At least 2 years of experience in a clinical or healthcare environment.
  • Experience with diverse populations and complex cases.
  • Strong communication and interpersonal skills.
  • Ability to function effectively within a multidisciplinary team.
