Position Overview

The Licensed Vocational Nurse (LVN) Case Manager plays a crucial role in addressing both clinical and non-clinical requirements of members with significant health challenges. This position emphasizes a systematic, community-oriented, interdisciplinary approach to high-touch, person-centered care management.

Key Responsibilities

The LVN Case Manager primarily engages with members, including adults and children, who face chronic health issues, homelessness, high rates of hospital admissions, substance abuse, behavioral health needs, or are transitioning from incarceration. The role includes:

  • Facilitating in-person and telephonic interactions with patients utilizing evidence-based methodologies to enhance collaboration with their healthcare teams.
  • Educating and guiding members in managing chronic illnesses through the establishment of SMART (specific, measurable, attainable, realistic, timely) objectives.
  • Collaboratively developing and executing tailored care plans that encompass the member's medical, behavioral, dental, and social requirements.
  • Participating actively in interdisciplinary team meetings to ensure cohesive care delivery.
  • Providing support to non-licensed ECM case managers as necessary.
  • Conducting meetings with patients in various settings, including clinics, homes, or community locations.
  • Delivering educational resources to enhance understanding of disease management, medication adherence, and other healthcare needs.
  • Assisting members and their families in navigating potential healthcare system challenges, including financial or social barriers.
  • Utilizing positive reinforcement and a flexible approach to engage a diverse population effectively.
  • Advocating for members with medical, behavioral, and treatment teams.
  • Connecting members with community resources to support their health and well-being.
  • Completing comprehensive assessments to evaluate member needs.
  • Promoting health and collaborating with members on their healthcare plans.
  • Documenting all services rendered accurately and promptly in the electronic medical record.
  • Following up on missed appointments, referrals, and discharges from hospitals or urgent care.
  • Coordinating appointments, sending reminders, and arranging transportation as needed.
  • Maintaining confidentiality in accordance with clinic policies and HIPAA regulations.
  • Fostering a respectful environment while collaborating with colleagues and supervisory staff.
  • Providing staff coverage as required.
Qualifications

Education: Must possess an active, current, unrestricted Licensed Vocational Nurse (LVN) license issued by the State of California.

Certification: Current CPR certification is required.

Experience: A minimum of two years in a clinical service delivery or managed care setting, specifically in a care coordination capacity.

Other Requirements: Valid driver's license and proof of state-required auto liability insurance are necessary.
