The Licensed Vocational Nurse (LVN) Case Manager plays a crucial role in addressing both clinical and non-clinical requirements of members with significant health challenges. This position emphasizes a systematic, community-oriented, interdisciplinary approach to care management, focusing on personalized and high-touch interactions.
Key Responsibilities- Engage with patients through in-person visits and telephonic communication, utilizing evidence-based strategies to foster collaboration with their healthcare teams.
- Guide and support members in managing chronic health conditions by establishing SMART (specific, measurable, attainable, realistic, timely) objectives.
- Collaboratively develop and execute tailored care plans that encompass the medical, behavioral, dental, and social needs of each member.
- Participate actively in interdisciplinary team meetings to ensure cohesive care delivery.
- Provide assistance to non-licensed ECM case managers as required.
- Conduct meetings with patients in various settings, including clinics, homes, or community locations.
- Deliver educational support to enhance understanding of disease management, medication adherence, and access to specialty services.
- Assist members and their families in navigating potential challenges related to the healthcare system, including financial or social barriers.
- Utilize positive reinforcement and adaptable strategies to effectively engage a diverse patient population.
- Advocate for members in collaboration with medical, behavioral, and treatment teams.
- Offer community resource information to members to support their health journey.
- Conduct comprehensive assessments to evaluate member needs.
- Promote health initiatives and collaborate with members on their healthcare plans.
- Accurately document all services provided in the electronic medical record for each patient.
- Follow up on missed appointments, referrals, and discharges from hospitals or urgent care facilities.
- Coordinate appointments, send reminders, and arrange transportation as necessary.
- Maintain confidentiality in line with clinic policies and HIPAA regulations.
- Demonstrate respect towards members and work collaboratively with colleagues and supervisory staff.
- Provide coverage for staff as needed.
Education: Must possess an active, unrestricted Licensed Vocational Nurse (LVN) license issued by the State of California.
Certification: Current CPR certification is required.
Experience: A minimum of two years in a clinical service delivery or managed care environment, specifically in a care coordination role.
Other Requirements: Valid driver's license and proof of state-required auto liability insurance are necessary.
