Job Overview

Program Overview:

The Enhanced Care Management (ECM) initiative is tailored to deliver holistic and organized care to Medi-Cal beneficiaries who face intricate health and social challenges, including individuals with multiple chronic illnesses, those experiencing homelessness, or individuals confronting substantial behavioral health issues. The primary goal is to enhance health outcomes through a personalized approach that encompasses intensive care management, addressing physical, behavioral, and social determinants of health.

Position Summary:

The ECM LCSW will be responsible for providing clinical oversight and supervision to the ECM non-clinical case management team and clinical personnel, including associate social workers and/or associate therapists. This role is pivotal in ensuring the provision of high-quality care while adhering to program, contractual, and ethical standards. The LCSW will support the organization's mission, serve as a clinical resource, and oversee the development and monitoring of care plans aimed at achieving optimal patient outcomes. Collaboration with the interdisciplinary team (ICT) is essential to support patients and engage their support systems in identifying priorities aligned with the member's needs and objectives. Additionally, the LCSW will deliver behavioral health therapy and therapeutic interventions to ECM patients.

Key Responsibilities:

  • Clinical Oversight: Provide guidance to non-clinical case managers and clinical supervision to associate social workers and/or associate therapists.
  • Act as a clinical resource for the ECM care team.
  • Ensure the delivery of services meets high standards of care.
  • Maintain compliance with program and contractual obligations.
  • Adhere to legal and ethical standards while meeting performance expectations.

Care Plan Management:

  • Oversee and evaluate care plans to ensure effective implementation and achievement of desired outcomes.
  • Recommend modifications to care plans as necessary to meet patient goals.

Patient and Family Support:

  • Provide psychoeducational resources to patients and their families or caregivers.
  • Deliver behavioral health therapy and therapeutic interventions.
  • Offer emotional support and counseling during crisis situations.
  • Facilitate communication and problem-solving to address immediate needs and concerns.

Training and Development:

  • Conduct training sessions for staff as required.
  • Engage in all assigned meetings and training activities.

Collaboration:

  • Work collaboratively with all staff and external partners.
  • Participate in interdisciplinary team meetings to ensure coordinated care.
  • Align closely with ECM administrators, implementing policies and procedures as directed.

Quality Assurance:

  • Implement quality assurance protocols, including record reviews.
  • Support and enforce program policies.
  • Monitor program performance and outcomes, making recommendations for enhancements.
  • Document all services rendered accurately and promptly in the electronic medical record.
  • Participate in assigned quality improvement initiatives.

Cultural Sensitivity:

  • Demonstrate sensitivity to the cultural and socioeconomic characteristics of the service population.

Safety and Compliance:

  • Perform duties safely and ensure adherence to safety practices.
  • Follow the National Association of Social Workers (NASW) Code of Ethics.
  • Comply with all policies, procedures, and legal mandates, including HIPAA and confidentiality regulations.

Education and Qualifications:

  • Master's degree in Social Work (MSW) from an accredited institution.
  • Current and unrestricted Clinical Social Worker (LCSW) license in California.
  • Eligibility to provide clinical supervision to Associate Social Workers (ASW) as per Board of Behavioral Sciences (BBS) guidelines.

Experience and Skills:

  • Minimum of 2 years of experience in a clinical or healthcare environment.
  • Experience with diverse populations and complex cases.
  • Strong communication and interpersonal skills.
  • Ability to function effectively within a multidisciplinary team.
