Position Overview:

We are looking for a committed Case Management Specialist to be part of our Pre-Filing Diversion Initiative. This position is centered on advocating for clients and coordinating vital services aimed at enhancing resilience and self-sufficiency among individuals enrolled in the program. The successful candidate will strive to achieve positive outcomes and empower clients in their personal journeys.

Key Responsibilities:

  • Maintain professionalism in all interactions
  • Support the mission of the Agency
  • Educate clients about available resources
  • Participate in training sessions and team meetings
  • Collaborate with colleagues on tailored treatment strategies
  • Provide vocational guidance and referrals
  • Deliver outreach and follow-up services
  • Assist in discharge planning
  • Document case management efforts
  • Perform additional duties as required

Qualifications and Experience:

  • Enrollment in a relevant academic program related to Addiction Studies
  • Minimum of 1 year experience in a Behavioral Healthcare environment
  • DVSS Certification for staff involved in DV-specific programs (HIV initiatives)
  • Bilingual in English and Spanish is preferred

  • Strong commitment to working with justice-involved individuals
  • Non-judgmental approach and positive regard for clients
  • Experience in providing field-based services
  • Excellent interpersonal abilities
  • Highly organized and detail-oriented
  • Proficient in Microsoft Office Suite
