Job Summary

We are seeking a highly skilled Case Manager II to join our team at Tarzana Treatment Center. As a key member of our behavioral healthcare team, you will be responsible for providing case management services to patients assigned to your caseload.

Key Responsibilities
  • Case Management: Conduct comprehensive assessments, develop and implement individualized treatment plans, and provide ongoing support to patients and their families.
  • Communication: Collaborate with multidisciplinary teams, including healthcare providers, social workers, and other stakeholders to ensure seamless care coordination.
  • Referral and Linkage: Identify and connect patients with community resources, services, and support groups to promote recovery and well-being.
  • Documentation: Maintain accurate and detailed records of patient interactions, progress, and outcomes.
  • Professional Development: Participate in ongoing education and training to enhance knowledge and skills in addiction treatment and case management.
Requirements
  • Education: Bachelor's degree in social work, psychology, or a related field.
  • Experience: 2 years of experience in a behavioral healthcare setting, preferably in addiction treatment.
  • Certification: Registration or certification with a DHCS-approved state-level counselor certification agency (for AOD programs).
  • Language: Bilingual English/Spanish preferred.
  • License: Valid California driver's license and insurance.
