Key Responsibilities:
- Conduct comprehensive assessments of patients' needs and develop individualized treatment plans to address their unique challenges.
- Provide education and support to patients and their families on topics such as health literacy, medication management, and disease prevention.
- Facilitate connections to community resources and services, including housing, employment, and mental health services.
- Collaborate with multidisciplinary teams to ensure seamless care coordination and communication.
- Document patient progress and outcomes in a timely and accurate manner.
- Participate in ongoing training and professional development to stay current on best practices and evidence-based interventions.
Requirements:
- 1 year of experience in a behavioral healthcare setting, preferably in a case management or patient advocacy role.
- Enrollment in an academic Addiction Studies program and registration with CCAPP, CAADE, or CADTP (for AOD programs).
- Bilingual English/Spanish preferred.
- Valid California driver's license and insurance.
Preferred Qualifications:
- Experience providing field-based services and working with diverse populations.
- Strong people skills and ability to work with people of various ages, racial, ethnic, cultural, educational, and socio-economic backgrounds.
- Organized, detail-oriented, and creative problem-solving skills.
- Proficient in computer skills and use of Microsoft products such as Outlook, Word, and Excel.
