- Utilize multiple clinical review systems effectively to manage tasks and ensure efficient workflow.

- Proficiency in data entry and familiarity with medical terminology.
Qualifications:
Applicants should possess:
- A High School diploma or GED.

In this position, you will:
- Provide outreach to members via phone to assist with care plans, community resources, and scheduling inquiries.
- Support members in connecting with health plan and community resources to ensure high-quality service.
- Document and maintain non-clinical member records in accordance with established practices.
- Understand local benefits and resources to address Social Determinants of Health (SDOH) needs.
