The Mental Health Practitioner specializing in Co-Occurring Disorders is responsible for delivering treatment services aimed at fostering recovery and community integration for individuals within the ACT team, utilizing an Integrated Dual Disorder care model. Key responsibilities include performing substance use assessments and facilitating both individual and group interventions. The Specialist provides cross-training and support to the team on effective practices to enhance engagement and sustain recovery, emphasizing harm reduction strategies.

  • Collaborate with clients to develop, implement, and adjust recovery goals and plans in accordance with ACT fidelity standards.
  • Conduct assessments, create treatment plans, and deliver substance abuse education to the team.
  • Lead the assessment and treatment of substance use issues, facilitate admissions to treatment programs, and connect clients with necessary resources.
  • Engage in psychiatric rehabilitation, support interventions, and coach clients on independent living skills.
  • Work alongside the Vocational Specialist to provide employment support and collaborate with the ACT nurse on wellness objectives.
  • Participate in court hearings, complete required documentation, and ensure proper filing of reports.
  • Function within a multidisciplinary team, engaging clients and their support networks.
  • Provide crisis intervention, transportation, and assist in discharge planning with clients.
  • Contribute to professional development, attend team meetings, and maintain thorough documentation.

Physical Requirements:

  • Ability to type, drive, sit, walk, climb stairs, and lift heavy objects.

Qualifications:

  • Bachelor's or Master's Degree in Behavioral Sciences or a related field.
  • A minimum of 2,000 hours of supervised experience in mental health.
  • A minimum of 2,000 hours of training or supervised experience in substance abuse treatment.
