Job Overview
As an integral part of the healthcare team, the RN care manager:

  • Ensures that the care delivered by the healthcare team is executed in an efficient and cost-effective manner while fostering optimal patient outcomes.

  • Assists the physician and healthcare team in advocating for cost-effective utilization of healthcare resources.
  • Guarantees that the patient is admitted to the suitable level of care and that services are rendered promptly by utilizing standardized criteria and adhering to federal and state regulations.

  • Collaborates with the patient, caregiver, and healthcare team to evaluate the discharge requirements of the patient and formulates/facilitates a secure and appropriate discharge plan.

Minimum Qualifications
Education
Graduate of a professionally accredited nursing program is required.
BSN is preferred.
Experience
3-5 years of experience in acute care or community nursing is required.
Licensure and/or Certification
Current license as a registered nurse in Massachusetts is required.
CCM is preferred.
