Key Responsibilities

  • Deliver high-quality patient care in a home setting, adhering to Emerson Hospital's standards and regulations.
  • Conduct thorough assessments of patients' physical and emotional needs, developing personalized care plans to address their unique requirements.
  • Collaborate with the patient's primary nurse and other healthcare professionals to ensure seamless care coordination.
  • Provide education and support to patients and their families on disease management, medication adherence, and healthy lifestyle habits.
  • Document patient progress, identifying areas for improvement and implementing changes as needed.
  • Participate in ongoing education and training to stay current on best practices in home care nursing.

Requirements

  • Education
    • Graduate of an approved practical/vocational nursing program.
  • Experience
    • Minimum two years of medical/surgical experience required.
    • Knowledge and experience in the practice of community health nursing preferred.
    • Home care experience preferred.
  • Licensure and/or Certification
    • Current MA license to practice as practical/vocational Nurse.
    • BLS healthcare provider required.

    • Critical thinking skills.
    • Organizational skills.
    • Computer skills.
    • Ability to maintain confidential information.
