Job Summary

As a Physical Therapist with Emerson Hospital, you will play a critical role in the Rehabilitation Team, responsible for delivering high-quality patient care in the home setting. Your expertise will be essential in evaluating, diagnosing, and intervening to develop and implement individualized care plans that meet the unique needs of each patient.


Key Responsibilities

  • Evaluate patients to identify physical therapy needs and develop comprehensive care plans
  • Deliver evidence-based interventions to improve patient function and mobility
  • Collaborate with multidisciplinary teams to ensure seamless care coordination
  • Provide education and support to patients and their families to promote independence and self-management
  • Participate in care management groups and education sessions to stay up-to-date on best practices


Requirements

  • Bachelor's Degree or higher from an accredited physical therapy program
  • MA PT license and BLS certification required within 30 days from hire
  • At least one year of experience in an acute and/or rehab setting, with prior home care experience preferred
  • Strong clinical skills, with ability to prioritize workload and respond to urgent needs
  • Excellent communication, assessment, critical thinking, and problem-solving skills
  • Ability to maintain confidential information and read, write, and communicate in English
