Care Manager

At MetroPlusHealth, we are seeking a highly skilled and compassionate Care Manager to join our team. As a Care Manager, you will play a critical role in optimizing members' healthcare and delivery of care experience, while achieving cost savings through improved quality of care.
Key Responsibilities:- Assess and evaluate members' needs, identifying opportunities for improvement and developing individualized plans of care.
- Collaborate with members' care teams to ensure seamless transitions of care and effective communication.
- Provide education and support to members on chronic disease management, preventive care, and medication adherence.
- Address social determinants of health issues and link members to available resources.
- Ensure accurate and comprehensive documentation in our Care Management System.
- Bachelor's Degree required.
- Minimum 2 years' experience in a healthcare setting, Care Management, or Managed Care setting.
- Ability to proficiently read and interpret medical records, claims data, pharmacy, lab reports, and prescriptions.
- Registered Nurse with current NYS license.
