Position Overview

Catalyst Health Group is a prominent healthcare provider dedicated to delivering exceptional medical services to our community. We prioritize high-quality, patient-focused care in a nurturing and supportive atmosphere.

We are currently in search of a Senior Services Care Coordinator to assist our senior clientele. In this role, you will facilitate access to various healthcare programs, including Medicare Advantage and traditional Medicare. Responsibilities will include conducting essential screening tests, managing documentation required by different health plans, and generating reports. This position entails significant patient interaction, both in-person and via telephone. Proficiency in Excel is advantageous, and a professional demeanor is essential.

This role will initially require 90 days of onsite work, after which a hybrid schedule (3 days onsite / 2 days remote) will be available.

Key Responsibilities:

  • Address inquiries from team members regarding the senior care demographic.
  • Perform Focus of Care Testing on patients, manage lab requisitions, and document results in patient records.
  • Process laboratory requisitions efficiently.
  • Conduct weekly inventory of medical supplies and submit orders to the Supervisor.
  • Compile reports for senior population data analysis using Excel and relevant insurance platforms.
  • Contact senior patients to arrange testing, appointments, and educational sessions.
  • Provide desktop support, ensuring all tasks are processed daily.
  • Offer coverage for fellow Care Coordinators as needed.
  • Coordinate training schedules for providers and team members with external vendors.
  • Disseminate communication emails to providers, team members, and external vendors.
  • Prepare educational materials for patients, providers, and team members.
  • Conduct necessary attestations.
  • Participate in meetings with team members and external vendors.
  • Occasional travel to various Catalyst Health Group locations may be required.
  • Engage in substantial patient contact both in-person and over the phone.
  • Perform additional administrative tasks as required.

Qualifications:

  • 2-3 years of relevant experience.
  • Active and current Certified Medical Assistant certification.
  • Experience with Athena EMR is a plus.
  • Proficiency in Excel is beneficial.
  • Professional, collaborative team player with a positive outlook.
  • Experience in managing confidential and sensitive information; understanding of HIPAA regulations.
  • Familiarity with basic desktop software, medical practice management, and electronic medical record systems.
