Community Living Options Coordinator

As a Community Living Options Coordinator, you will play a vital role in supporting clients within their homes through various community programs. Your responsibilities will encompass assessing and managing client needs, ensuring they receive the necessary support and resources.
Key Responsibilities:
- Conduct thorough in-home assessments and periodic reassessments of clients.
- Maintain regular communication with clients to monitor their progress and needs.
- Develop and update individualized care plans annually, making adjustments as required.
- Accurately document case management hours in the designated tracking system.

Qualifications:
To be considered for this role, candidates should possess:
- A Bachelor’s degree in Social Work, Psychology, Gerontology, Sociology, Counseling, Nursing, or a related field from an accredited institution; or
- A Bachelor’s degree in an unrelated field with at least two years of experience working with older adults or individuals with disabilities; or
- A minimum of two years of college education combined with two years of relevant experience in social work or a related area.
Strong organizational, interpersonal, and decision-making skills are essential. A valid driver’s license is also required.
