Job Summary

Project FIND is seeking a dedicated and compassionate Social Worker to provide comprehensive supportive services to older adult clients at our centers. As a key member of our team, you will be responsible for assessing client needs, coordinating services, and providing counseling to ensure the best possible outcomes.

Key Responsibilities
  • Client Assessment and Service Coordination: Conduct thorough assessments of older adult clients to identify their needs and develop personalized service plans.
  • Counseling and Support: Provide counseling and support to clients to help them navigate the challenges of aging and access available resources.
  • Community Outreach: Conduct outreach and intake of older adult center participants to ensure they are aware of available services and benefits.
  • Benefits and Entitlements: Screen clients for benefits and entitlements, assess eligibility, and assist with applications and follow-up.
  • Resource Referral: Provide information and referral to other appropriate agencies and community resources as needed.
  • Conflict Resolution: Resolve and/or mediate potential conflict situations involving center participants.
  • Record Keeping: Maintain accurate and up-to-date client case records, work logs, and statistical reports.
Requirements
  • Education and Experience: Bachelor's degree in Social Work or Gerontology and two years' experience in casework or social services.
  • Case Assistance Experience: Possess case assistance experience and knowledge of benefit and entitlement programs for older adults.
  • Skills and Abilities: Ability to work independently, accomplish work responsibilities with minimum supervision, and perform multiple tasks with strict timelines.
  • Language Skills: Bilingual in English and Spanish language is preferred.
Work Environment

This is a full-time position, Monday-Friday, 9am-5pm, with occasional weekend and holiday availability. Project FIND participates in E-Verify and requires the ability to work in a fast-paced, dynamic environment.
