Social Services Director

Alaris Health at St. Mary's is seeking a highly skilled and compassionate Social Services Director to lead our team in providing exceptional care and support to our residents. As a key member of our interdisciplinary team, you will be responsible for planning and administering social service programs that promote the well-being and independence of our residents.
Key Responsibilities:
- Program Development and Implementation: Develop and implement social service programs that meet the unique needs of our residents, including transition planning, community outreach, and resident advocacy.
- Staff Supervision and Development: Supervise and mentor a team of social services staff, providing guidance and support to ensure excellent service delivery.
- Community Partnerships: Establish and maintain partnerships with community agencies and organizations to access resources and services that benefit our residents.
- Resident Care Planning: Collaborate with the interdisciplinary team to develop and implement individualized care plans that address the physical, emotional, and social needs of our residents.
- Communication and Liaison: Serve as a liaison between residents, families, and external agencies to ensure seamless communication and coordination of care.
Requirements:
- Education: Bachelor's degree in social work or a related field (master's degree preferred)
- Licensure: Licensed Clinical Social Worker (LCSW) in the state of New Jersey
- Skills: Excellent interpersonal and communication skills, with the ability to work effectively with diverse populations and stakeholders
- Knowledge: Strong knowledge of social services regulations, policies, and best practices
- Experience: Proven experience in social services leadership, with a focus on program development, staff supervision, and community outreach
